Volunteers

This volunteer application form must be submitted once each year. The AYSO year starts on August 1st, so if you volunteer in the fall your form does not have to be submitted again in the winter or spring.


  1. Go to www.eayso.org

  2. Sign up with your legal name and register with eayso.org to obtain a log-in I.D.

  3. Log in again after that process is finished and "Add a Volunteer".

  4. Fill in the blank pages with your information (use legal name). We are Region 473.

  5. Complete professional and personal references.
  6. Complete the Disclosure section.
  7. While it says signature on page 7 of 7, this is a temporary signature – a hard copy with a wet signature is still required.

  8. When finished, go to Print Form at the top of the page.

  9. Bring 2 copies of the forms along with a government photo I.D. to the Safe Haven course for verification of your id.

  10. If you want to keep one for your own records, print an additional copy.

  11. The Instructor will give the forms to the Child and Volunteer Protection Advocate.
